Create Custom search to list multiple values in contact custom groups

This will allow folks to export multiple value data. The custom search should allow users to choose one or more custom groups that have multiple values and the ability to export them.

At some stage we might want the ability to add what field in each custom group and also allow custom groups that are not multi-valued.
