reports improvements / fixes

1. reports currently do NOT respect transaction status (i.e. the totals include Failed and Cancelled and Pending contribution). Quick solution is to include a contribution_status_id filter and make it completed be default. Fix is to be included in all contribution reports.

1. Sample data generator for Soft credits is setting civicrm_contribution.contact id = civicrm_contribution_soft.contact_id (for the same contribution). This is nonesensical and makes the sample report data LOOK wrong (Soft Credit Name = Constituent Name for all rows). Generate code should be fixed.
