Contact display_name and sort_name not populated from email-only anonymous user online contribution OR membership signup

Update configuration for sample contribution page 2 (Membership Signup and Renewal) to allow Pay Later.
Signup for membership as an anonymous user.

• the contact, membership, and contribution records are created but display and sort_name columns are not populated as expected. Hence they show up in Find Membership and Find Contributions selectors with empty "Name" column values.
