Add location type fields to Profile

Similar to import/export, CiviCRM Profile should allow the user to pick a specific location type and phone type for the various location fields

The default option of primary should also exist.

Again most of this code already exists, so we should reuse. Might be nice to see if we can consolidate the import/export mapping code and clean that up while we are doing this
