cannot import .CSV file into contacts

OpenOffice .csv file will not load. I also created a MySQL table with all of these rows, with the column types as straight varchars, but using "Select * from ungiwg_import" fails to get to step "2" of the import process.

The .csv does get to step "3", however, during the import process, it is apparently stepping on a landmine, and resetting itself back to step 1.
