Can't save new contact if matching contact is found

      Description

      3.0 beta 5 - when I tried to create a contact where another (different) contact with a matching name was found, the system prompted me to click the 'Save Matching Contact' button if I really wanted to create a second contact with the same name. The button does not display, and clicking the regular 'save' or 'save and new' buttons just brings back the same screen.
