convert CRM/Contribute/Form/Contribution/Main.tpl - use divs, add classes

remove table based layouts and replace with format:

<div class="label">

</div>
<div class="content">

</div>
</div>

for all elements that get put through Main.tpl contribution page

add identifying classes to fieldsets/groups etc...
