Some autocomplete contact boxes don't honor the search settings

1. Migrate existing autocomplete to configurable autocomplete.
2. Remove address part which is show below autocomplete

Suppose the search settings page is set to include email and phone for autocomplete.
On the email form, the autocomplete only shows email and does not show phone.
The current employer field on the individual form seems to always show only city.
