Import Contacts Preview - Simplify UI for Importing into New and/or Existing Groups

(This is probably a good task for Manish or Deepak)

On step 3 of the Import Contacts Wizard:

• Remove the checkbox field for "Create a new group from imported records"
• Replace with a link: "» Create a New Group from Imported Records"
• Put the newGroupName and newGroupDesc fields in a <div> that is hidden by default. Unhide them when the above link is clicked.
• Modify form processing to create group and put contacts in it IF newGroupName field has a valid value.
• Add a second link: "» Join New Contacts to Existing Group(s)"
• Put groups field in another hidden div. Unhide when link above is clicked.
• Form processing for this field doesn't need to be modified (it already does the processing based on one or more values being selected).
