Form markup and styling improvements.

• Create consistent style for input forms - http://files.rayogram.com/nyss/design/100305/11.html
• Replace collapsible fieldsets, and jQuery ui-accordion widget with a simple new crm-accordion widget
• Button restyling with embedded icons
• Implement rounded corners for all browsers using 'dd roundies'
• Add a 'Clear' form link style to clear / unselect radio buttons (Advanced Search radio buttons, custom field radio buttons, date fields)
• Replace status icons and cleanup status message layout (status.tpl)
• Replace contact icons
