Skip NULL fields on dedupe

Dedupe currently does not skip rows containing NULL values, but skipping them would be beneficial in many cases (e.g. dedupe over organisations which skips individual rows in the civicrm_contact table).

The attached patch from http://forum.civicrm.org/?topic=13741#msg58693 implements this.
