last create contrib page bug

            Description

            The last page for creating a new contribution page was not formatted properly and the Previous and Continue buttons weren't active. See attached file. Other links seemed live. Weird placement of http://sandbox.openngo.org/civicrm/drupal-php5/civicrm/admin/uf/group?reset=1&action=browse below the title.
