Custom Contact Type not upgraded successfully and website missing from contact Edit form

      Description

      After upgrading a 3.1.3 site to 3.2 Beta 2, the Contact edit page has no Website field and when trying to create a new Contact using a Custom Contact Type based on Organisation that we created under 3.1, the only thing that displays is the Website title with the "add" link, but there is no text entry box and clicking on "add" doesn't display one.
