Online membership signup / renewal form is not working

            Description

            When I click "Confirm" to submit the form (using the sample "Member Signup and Renwal" online contrib page id = 2), the form just reloads (doesn't progress to the next page). I'm using the Dummy processor - but pretty sure that's not the issue since I just tested contrib page 1 (the one w/o membership block) w/ Dummy processor and it works fine.
