Extra options for CiviEngage walklist

Patch adds extra options to CiviEngage walklist

• last contribution date
• last contribution amount

• admin settings form allows the following things to be set:
• number of rows per group
• whether to group on zip code
  -whether to group on odd/even
• whether to do a new page per group
• option to change last contribution date to text (e.g. do not canvass) if the person has donated within configurable number of months - e.g. the last contribution column will show 'do not canvass' in the printed version instead of a date if they have donated within the last 18 months

Note that you have to give drupal permission to allow access to admin setting form.
