hook_civicrm_links improvements

hook_civicrm_links currently is defined as taking four parameters, but the fourth ($links) is not passed to the hook and thus throws an error when the hook is defined.

the hook should also allow the modification of existing links – not just the addition of new ones, as is the current behavior.
