CiviCampaign improvement / fixes.

We should implement / fix following points :

• show / hide on survey form for result set still not right - In 'add' mode, the Select Result Set field is shown when [ x ] Create new is selected. Should only show this select when user selects "Use existing".

• when there are two result sets, Record Survey Responses picks the "wrong" one (last one added?)

• Survey Instructions (from add /edit survey form) should be displayed at top of Interview Respondents page.

• Conduct Survey - show of Edit Search Criteria doesn't work (won't open) if you switch to Interview Tab and then switch back to Reserve tab

• Campaign-related activity types (Survey, Canvass...) are missing from Activity Report activity type filter selections

• Duplicate sort icons show on table headers (activity report, record survey responses...)

• Disable custom data set - the dialog button has a styling problem (cancel is all black, and ok is black on mouseover) - probably true for all instances of this disable / enable dialog.

• implement Recontact Interval in Find Respondents to Reserve interface. This means that search should filter out respondents w/ Completed activity UNLESS the recontact interval in their activity's Result is expired.
