Allow admins to assign an Online Contribution Page to an offline contribution and display assigned contribution page when viewing contributions

            Description

            Sometimes staff members need to record a contribution "offline" but assign it to an online contribution page (primarily so it shows up on a contribution page widget). It is also helpful when viewing a contribution to be able to see whether it is linked to an online contribution page.

            Add a drop-down to select Online Contribution Page to the "Additional Information" pane in the New / Edit Contribution form.
            Conditionally add display field to View Contribution - Online Contribution Page (displays the title).
