      Description

      1. Create a new report which can be used both to "collect" survey responses on paper (Walklist or offline Phone-banking) AND to report on responses for contacts who have been interviewed.

      2. The report can display Survey Result column (Result set options - e.g. Completed, Moved, Refused ...) as well as Survey Responses (custom Activity fields) as configured in the Survey's profile

      3. The report can be run for "Reserved" respondents - and printed for use in collecting survey responses on paper. Or it can be run for "interviewed" respondents.

      4. Print and PDF outputs will include a "cover sheet" which provides a legend mapping result set labels to values (codes) and survey response multiple choice question "codes". When "Reserved Respondents" are displayed, the result columns will include all the available multiple choice options - allowing the interviewer to simply "mark" the response on a paper copy of the report.

      5. Print and PDF outputs have Group By options for Street Name and Odd / Even Street Number. If checked, the report will break to a new page for each street and also put odd street numbers on a separate page from even street numbers (designed for Walklist support).
