Die with a fatal error if we fail to create a contact

for some strange/stupid/absurd reason, we decide in our infinite wisdom to keep moving along in contact creation without a contact!!

This can spin out of control and result in issues like the one reported here:

Better to save the user and us a lot of grief by dying with a fatal rather than just continuing. We had a fatal, but someone commented it out. Furhter investigation revealed that I was that idiot who commented it out. In my defense, we did it so the test suite would not abort!

Am in talks with the test folks to see how we can address this case
