CiviCampaign Admin menu items were not inserted during 3.2 to 3.3 upgrades

      Description

      The new CiviCampaign admin menu and it's sub-menus were not included in the 3.2 -> 3.3 upgrade scripts and so are missing from sites upgraded to 3.3 or 3.4 from 3.2. In order to fix this (and add the the Engagement Level options menu) - we need to delete these menu items if they exist (sites that started in 3.3) and then reinsert for all sites in the proper location.

      The behavior reported by Sarah was due to Administer > CiviCampaign menu item not existing on that site.

      After upgrading CiviCRM 3.3.5 to CiviCRM 3.4.0 (running under Drupal 6), the new menu item "Engagement Index" appears as a top level menu item. Its in between "Search" and "Contacts"

      I manually adjusted the navigation menu to nest it under "Administrator --> Option Lists", but I am not sure where it logically belongs.
