Manage CiviCRM Extensions menu missing on upgraded sites

            Description

            The navigation menu item "Administer --> Customize --> Manage CiviCRM Extensions" is missing from my CiviCRM v. 3.4.0 environment that was upgraded from CiviCRM 3.3.5.

            This menu item was added during 3.3 release cycle and will be present in sites installed for 3.3.x but not sites upgrade from 3.2 and earlier. The 3.4.1 upgrade will insert for all sites.
