            Description

            Add same code as in CRM/Upgrade/Incremental/ThreeThree.php to ensure phone_ext column is created if it's not already present.

            NOTE: This is needed because we added the column for NYSS in 3.3.7 after 3.4 / 4.0 release. This work should be committed for the same point release as when 3.3.7 is merged to 3.4 branch.
