Description
Currently we don't set a 'name' value for the default rules and we don't require it when editing or creating new rules. Now that we are exposing select drop-downs for folks to choose a rule - it makes sense to have names that folks can use to differentiate between rules.
For upgrades, we can do something like this:
update ... set name = contact_type + level + id
if name is null
