Custom data stored in core tables

This patch is a functional proof of concept, not a complete implementation. I have changed custom field logic to alter the schema of the core table being extended. So for example, a custom field extending Event would become a column on the civicrm_event table.

The attachment can create simple columns, assign and retrieve values. Many cases are not going to be handled correctly.
