Add 'Paid By' filter to bookkeeping report

A very small patch that allows transactions to be filtered by payment instrument. I find this quite useful when e.g. reconciling a cash box or preparing checks for deposit, etc. Also provides the only method I could discover of identifying transactions where payment instrument is NULL.
