Add an Activity when Membership Type updated in backend

Currently changes to the membership type of a membership made during renewals are recorded and can be seen by reviewing the Activities for a contact. Changes made to membership type of memberships are not similarly recorded, leaving gaps in the history of a membership. The attached patch files address this issue.
