Notes - add Subject field

            Description

            Add a "Subject" column/field to Notes class (civicrm_note). This field is optional, VARCHAR 255. Should be displayed on Notes selector(s) and included in view and edit pages/forms.

            NOTE: We will also make this property "searchable" as part of the Query Builder functionality (covered in a separate issue).
