merge households on export only uses "member of household" relationship type and merge is broken due to change to radio buttons

When exporting contacts, we have the option of merging households. However, current functionality only merges based on the "member of household" relationship type, where it should be merging based on that and the "head of household" type.

I have this fixed and will supply a patch shortly (I need to port to 4.1).
