Printer - friendly view broken on report instances

The printer friendly icon on report pages doesn't work properly because it doesn't get the instance value from the url. Attached is a patch created by Darshan More for a JMA client, the United Church of Canada.

The Print Report button provides a work-around. However, we will shortly be provide a patch so that it invokes the browser's print dialog, which is the expected action for a 'Print' function on a website, at least these days.
