update Serbia and Montenegro provinces

currently we have four states/provinces assigned to "Serbia and Montenegro"
we also have Serbia and Montenegro broken out into separate countries (as they should). the provinces should be assigned to the respective country rather than the no longer existent combined country. not sure if we should also remove the combined country from the country table.

i think it's safe to base the update on ids, as those values should be consistent across all installations. but someone should confirm that.
