limitation on number of subtypes used for custom field

civicrm_custom_groups.extends_entity_column_value
is currently set to varchar(64). that length is inadequate as we store the name value (not the id) for contact subtypes and other value types for custom data groups. with the current length you can be unexpectedly limited in how many selections are made.
