Add custom field to HR's data model but disable by default.

We need an extra field (with the option to attach evidence) to allow the recording of the Sponsor’s Certificate number that may be associated with the Visa reference number. This is commonly used in the UK as the Sponsoring organisation creates the Certificate for each Visa application via the UK Border Agency website.
