Skill Self-Assessment Widget

We need a new profile widget to collect the degree of mastery of a skill using a slider.
Data-type is a multi-select. User-interface-client will wrap this such that e.g. - selecting a 3, will multi-select 1-3, so that a volunteer of level 3, will automatically be matched for searches for level 2.

By default, 5 levels will be available. This could be user-configurable (Option Group).

Widget will replace the multi-select with a audio-pot-style fader/slider.
Labels should be visible in some way, perhaps tooltip as you slide.

Note: this is based on crew-connect for javascript libs... but also probably old, so more inspirational.
