Description
When more than one quickform exists on the page, (eg advanced search) we get conflicting ids for radio elements.
Reproduced on demo site.
1. go to advanced search
2. open "Change Log"
3. click the word label "Added" (not the radio button)
4. Notice how at the top, the CMS user is checked instead, not the added radio button for the label
