Membership block for quickConfig pricesets is broken

The problem is revealed after applying the patch for CRM-13827. The radio buttons are not added to the form for quickConfig forms where the membership priceset is set. Once the radio buttons are added, some extra work is required to ensure that the form validates properly and that totals are calculated correctly.
