Entity ref find / add contact widget broken for additional addresses

The widget that allows you to select or add a contact to "share address with" works beautifully on the initial address pane. However, if you click Another Address and try to 'share address' the widget is broken (see screenshot).

Webtest faiilure ; WebTest_Contact_AddTest::testIndividualAddWithSharedAddress revealed this bug when I was trying to fix it.

Coleman: Also noted that the change to use the new crm-status-box-msg for success messages (and the change in the success message text) also caused breakages in this webtest (filing a PR with fixes in this test - BUT I suspect several other web tests are affected).
