Expose filter options to EntityRef widget

This change adds a 'Refine search' option to the widget, which is used wherever users select a contact w/in a form - for example, Add Relationship, Add Activity, Add Contribution (from the 'Create New' button), etc. (screenshot attached). This allows the user to limit autocomplete search results to a specific contact type, group, tag, state, country, etc.
