"Tab with table" custom data style fields do not show up when adding a new record

      Description

      To reproduce:

      1. Added a custom data with "tab with table" style for "Individual" and my own custom contact type.
      2. Added a few custom fields.
      3. Goto newly created tab in the contact summary page, click to add a new record, but none of the fields appear. Just the save, save and new, and cancel buttons.
