CPS 07: College Prep Scholarship Section

Implement the College Prep Scholarship section of the Application as per the mockups at:
http://www.questbridge.org/app_staging/cps_07/cps_ranking.html
and
http://www.questbridge.org/app_staging/cps_07/cm_ranking.html

1. Add DB fields as needed for the "College Prep Scholarshp" form to Student.xml
2. Modify the College StateMachine - replacing the ForwardApp form w/ the new CollegePrep form
3. Create the php and tpl for CollegePrep form
4. Verify that College Ranking form submit handles task_status assignment for this "section" of the app, and redirects to the Submit Application page (which s/b the "next" page).
