Should not force default state/province when no default country selected

A default country selection is not always appropriate (eg for an international organization not wanting to privilege one country), and even when it is a default state/province selection may not be appropriate (eg for a national organization not wanting to privilege one province or state). Currently there is an error if no country is selected as default, as no state/province can be selected, but one is demanded. See image from d45 site running 4.5.5
