schema differences between upgrades and install

There are some discrepancies between the schema of upgraded database and the schema of new databases. Most of these differences are trivial and the error is with the upgrade script, not the new database schema, so probably not worth fixing.

A few changes seem to be worth making - that includes the 4.5 upgrade scripts (since many users may not have upgraded yet) and examples where the new database schema doesn't match the upgrade and should be.
