Activity Details report: add source, target and assignee phone number columns

1. Add the following columns to existing Activities (activity details) report:

• Source Phone
• Target Phone
• Assignee Phone

2. Let's simplify the other column labels by removing 'Contact' from string:
e.g.'Source Contact Name' => 'Source Name'; 'Target Contact Email' => 'Target Email'
