Location type does not show for custom address fields

      Description

      Steps to reproduce:

      1. Create a custom address group and field
      2. Add custom address field to the default Name and Address profile
      3. Note when you first add it, you have the option to choose location type
      4. Once added, click the 'edit' button and note that the location type menu is no longer visible.

      Appears to be a js bug.
