After CRM-3404 fix can no longer add additional locations

            Description

            After this fix http://issues.civicrm.org/jira/browse/CRM-3404 to fix demographics falling under additional location I no longer see the additional location fieldset to add a new location for a contact. I have verified on the demo server which you can see here. Click Primary Location and you should be able to scroll down and add additional locations. It seems to be missing now.
