Implement new Contact Add/Edit Interface as per following mock-up

• Need to implement new UI
• Need to restructure code for processing various blocks.
• If possible, structure the address fields section in the template so that developers can change field order without modifying PHP code.
• Groups listing box should be alphabetical by group title (currently sorted by group id). Test page with long group names and descriptions - and apply a max-width to the groups listing box style if necessary to prevent tags from being pushed off right side of page.

• Also fix Event Location and Domain Location according to new structure.
