Add a new requirement that all new contacts *must* be added to a group

Add a new requirement that all new contacts must be added to a group. The default will be the "master" group of the site/org that that user is responsible for. Sub-groups should also appear as options to add the contact to, but there should be no way (in the view or the controller) to add a contact w/o also adding it to at least one group.
