Standalone Form HTML for Profiles has relative URL

            Description

            In 2.0, standalone form HTML had an absolute URL for the form action, and so worked irrespective of the location of the form being submitted.

            In 2.2, the action URL for the form is relative. This means that the copied HTML won't work on another site unless it's modified first.
