CRM-5425 Clicking Pay Later does not then show the Address/Notes field

            Description

            It is not working on our 3.0.2 nor the sandbox http://sandbox.civicrm.org/civicrm/contribute/transact?reset=1&id=4
            If you click the pay later it just does not show the extra info.
            Same with http://www.fullercenter.org/civicrm/admin/contribute?reset=1&action=update&id=1&subPage=Amount

                I will turn this to a feature request to maybe do both?

                These 448 issues have not been worked on for the past 18 months.

                Doing a bulk close of old issues to make the issue queue more manageable. We should do this on a periodic basis.
