Link to Create Mode from profile fields view

It's great having the link from the profile list to use a profile in create mode; even better would be to have a button from the profile's fields page.

This is a simple fix to smooth the common workflow of
1. create the profile
2. add fields
3. get the link for people to use it.

It adds a "Use this profile in Create Mode" button on the profile fields page and adds a new icon class of "icon-play" showing the little triangle play button like a tape recorder.
